Fitness Evaluation


The KDAVIS FITNESS 90 TO FIT Program is a well-rounded and challenging program. In three 4-week blocks, you will work on improving your overall fitness level. You will focus on building strength, endurance, balance, coordination and target all muscle groups and energy systems. You will challenge your mind and body to step outside of your comfort zone. It is proven that workout programs that incorporate both variety and structure have the highest retention and satisfaction rate. No matter what your fitness level, your workouts will always be versatile, with a variety of exercises and training methods.

  • Three 4- week progressive workout programs which include a weight training (gym), bodyweight training (home) and cardio component and video demonstrations for all exercises

KDAVIS FITNESS wants to ensure your safety before starting any of our Training & Lifestyle Programs. Please answer these questions with honesty and to the best of your ability.

Has your doctor ever said that you have a heart condition that you should only do physical activity recommend by a doctor?
Do you feel pain in your chest when you do physical activity?
In the past month, have you had chest pain when you were not doing physical activity?
Do you lose your balance because of dizziness or do you ever lose consciousness?
Do you have bone or joint problems (ex. back, knee or hip) that could be made worse by a change in your physical activity?
Is your doctor currently prescribing drugs for you blood pressure or heart condition?
Do you know of any other reason why you should not do physical activity?

By purchasing this program I understand and am aware that physical-fitness activities are potentially hazardous activities. Although Kayla Davis has taken precautions in creating this document to ensure that I understand correct form/execution of exercises, I expressly assume and accept sole responsibility for my safety. I release Kayla Davis from any and all liability and claims for any injuries, disability, death or damage to any person incurred while participating in the program, such having occurred due to or in part due to any and all known or unknown medical conditions, injuries, illnesses, ailments, allergies, or medications.
